2.  If paying by credit card:

NOTE: Only forms with credit card payment will be accepted by fax.  Forms faxed without credit card payment will not be accepted.

CANCELLATION POLICY -- For notification of cancellation received at the AAI office:

  • Prior to May 12, amount paid less $75 administrative fee
  • May 12 - May 29, 50% refund
  • May 30 and thereafter, no refund
